*You are responsible for payment during each session. Please verify coverage with your specific insurance provider prior to your first appointment. Still Meadow Counseling is happy to bill in network insurance carriers.

 

Copays are due at the time of service. All fees may be paid in cash, check, credit card or HSA card. Checks may be payable to Still Meadow Counseling, LLC. If you do not wish to utilize your insurance, you are responsible for fees at the time of service.

Cancellation Policy​​

24 hours notice is required to cancel or change and appointment. A cancellation fee of $50 will be charged for cancellations with less than 24 hours notice or failure to show.

Notice of Right to a Good Faith Estimate

The No Surprises Act is a federal law which provides you with the right to a good faith estimate of the cost of my services upon request. In addition, The Consent to Services form supplied to you prior to treatment provides the actual cost of my charges in a written informed consent form to which you must agree prior to my providing services. The informed consent form will be available to you prior to you being seen for services and prior to any billing. In most cases it is impossible to estimate how many sessions you will need, and that will not be determined until your concerns are evaluated and will also vary based on the progress that you make, which depends in part on your efforts with the process. You will be free to discontinue services at any time or the services may otherwise be terminated in accordance with the informed consent form language.
